We’ve reviewed the informed approval and signed authorization process.

There are exceptions which allow for disclosure without the person’s specific approval. The person must be informed of these exceptions when confidentiality is explained. The person should also be informed of disclosures made without their approval whenever possible.

Best practice is to have a signed authorization whenever possible.
